Biden\u2019s office said the cancer was aggressive, had spread to the bone and appeared hormone-sensitive, meaning it may respond to treatment that targets hormones. He later began radiation and hormone therapy as part of his care.<\/p>\n\n\n\n

For men following the news, the bigger lesson is not only about one high-profile patient. It is about how prostate cancer is found, why some cases are discovered late and why the next steps can vary so much from one man to another.<\/p>\n\n\n\n

Some prostate tumours grow slowly and may be monitored for years. Others spread before they are detected or require treatment soon after diagnosis.<\/p>\n\n\n\n

Why diagnosis is only the first step<\/h2>\n\n\n\n

Prostate cancer is not one single disease.<\/h3>\n\n\n\n

A diagnosis can mean very different things depending on the stage of the tumour, how aggressive it appears to be and whether it has moved beyond the prostate. That is why doctors usually need more than one test before they can understand the full picture.<\/p>\n\n\n\n

PSA levels, imaging, biopsy results and staging scans can all help doctors determine whether cancer appears confined to the prostate or has reached nearby tissue, lymph nodes or distant sites such as bone.<\/p>\n\n\n\n

That distinction matters. A low-risk tumour found early may lead to active surveillance, where doctors monitor the cancer closely rather than treating it immediately. A more advanced cancer may require treatment aimed at controlling the disease while protecting quality of life where possible.<\/p>\n\n\n\n

The numbers men should know<\/h3>\n\n\n\n

Globally, prostate cancer remains one of the most common cancers in men. Recent estimates show around 1.47 million new prostate cancer cases worldwide in 2022, with the global burden projected to rise to 2.4 million new cases annually by 2040.<\/p>\n\n\n\n

Advanced disease is also rising. Distant-stage prostate cancer has been increasing across age groups, with reported annual increases of roughly 2.6% to 6.2%<\/a> depending on age.<\/p>\n\n\n\n

Survival also depends heavily on stage. When prostate cancer is found while still localised or regional, outcomes are far better. Once it has spread to distant parts of the body, five-year survival is much lower.<\/p>\n\n\n\n

These figures point to the same message: early detection matters, but so does precision. The goal is not simply to find more prostate cancer. It is to find the cancers that matter and avoid unnecessary treatment where monitoring is more appropriate.<\/p>\n\n\n\n

PSA can help, but it is not the whole answer<\/h3>\n\n\n\n

The strongest established risk factors for prostate cancer include age, family history and African ancestry or genetic susceptibility. The disease is rare in men younger than 40, but risk rises sharply after 50 and is most commonly diagnosed in older men.<\/p>\n\n\n\n

PSA testing can be a useful warning signal, but it is not a diagnosis by itself. A raised PSA may lead to further investigation, but it can also identify slow-growing cancers that may never become dangerous. That is one reason screening remains debated.<\/p>\n\n\n\n

For men with rising PSA levels, urinary changes, previous prostate concerns or a family history of prostate cancer, the next step should be a proper medical discussion. Why advanced cases can still be found late<\/h3>\n\n\n\n

Prof. Dr. mult. Michael K. Stehling, founder and medical director of VITUS Privatklinik<\/a> in Offenbach, Germany, said Biden\u2019s diagnosis should prompt broader awareness of how aggressive prostate cancer can present.<\/p>\n\n\n\n

\u201cThe news that former U.S. President Joe Biden has been diagnosed with advanced metastatic prostate cancer once again makes clear: prostate cancer can remain undetected for a long time \u2014 even in individuals under close medical supervision \u2014 and may present in an aggressive form,\u201d Stehling said.<\/p>\n\n\n\n

That is why diagnosis should not stop at confirming cancer is present. Doctors also need to understand how far it has progressed, how aggressive it appears to be and whether it has moved beyond the prostate.<\/p>\n\n\n\n

In advanced or complex cases, treatment planning may take into account the tumour stage, previous therapies, molecular characteristics and the patient\u2019s overall health.<\/p>\n\n\n\n

\u201cEspecially in advanced stages, more than standard therapy is needed today. What is decisive is a precise, individually tailored treatment concept that takes into account the biology of the individual tumor, the tumor microenvironment, and the patient\u2019s immune response,\u201d Stehling said.<\/p>\n\n\n\n

The quality-of-life question<\/h2>\n\n\n\n

Prostate cancer treatment decisions also involve deeply personal concerns.<\/h3>\n\n\n\n

The prostate sits close to nerves, blood vessels, the urethra, bladder neck and rectum. Because of that anatomy, urinary and sexual function often become part of treatment discussions, especially when men are comparing different options after diagnosis.<\/p>\n\n\n\n

For some patients, the goal is not only to treat the cancer, but to understand how each option may affect daily life. That can be especially important when cancer returns after previous treatment, when tumours are close to sensitive structures or when several treatment paths are being considered.<\/p>\n\n\n\n

Focal approaches, including electroporation-based techniques, are part of that wider conversation in selected patients and selected tumour patterns. They are not a replacement for all established treatments, and surgery, radiation and systemic therapy remain central parts of prostate cancer care, particularly in advanced disease.<\/p>\n\n\n\n

The clinic emphasises that its published findings are single-institution retrospective analyses, not randomised comparative trials.<\/p>\n\n\n\n

What men should take from Biden\u2019s diagnosis<\/h2>\n\n\n\n

The lesson from a high-profile advanced diagnosis is not that one treatment approach fits every patient.<\/p>\n\n\n\n

It is that prostate cancer can lead to very different decisions depending on when it is found, how it behaves and whether it has spread.<\/p>\n\n\n\n

For men, especially those with a family history, rising PSA levels or urinary symptoms, the practical takeaway is simple: do not treat prostate cancer as one uniform condition.
